How do I add this effect to a MIDI in ableton? Do I drag it in to the piano roll?

For transformation patches it's a bit different, you have to place them in a location such as "ableton\user\User Library\MIDI Tools\Max Transformations" and then go and find them in the "transformation" menu from a clip view.
